I confirm that my permit will be valid on the date of the road test.

I confirm that I have completed full 32-hours of the Parent Taught classroom/online course (Cert should be labeled For Driver License Only).

I confirm that I received my permit at least 6 months prior to the road test date.

I confirm that I have completed the IMPACT Texas Teen Driver (ITTD) Course.

I confirm that I have completed 30 hours of driving with an authorized adult.

I confirm that I have completed 7 additional hours of driving and 7 hours of observation with an authorized adult or a driving school.

IMPORTANT: All documents must be HAND SIGNED. Incomplete, missing, or incorrectly filled out documents will require rescheduling the road test at an additional fee of $45.

Each document must be uploaded an an individual file. You can upload multiple individual files at once.

IMPORTANT: You must bring hard copies of above required documents with you to the road test.

I confirm that all the information provided is correct. I understand that missing, incorrect, or incomplete information will result in rescheduling of the road test at an additional cost of $45. Furthermore, I recognize that an application with missing, incorrect, or incomplete documents will be rejected by DPS.
